He is survived by: his children, Mary Jo Hazel (Nick) of Campbellsville and David Shaw (Sue) of Lexington; his grandchildren, Matthew Hazel (Ashley) of Campbellsville, Justin Hazel (Taylor) of Campbellsville, Carlie Hazel of Campbellsville, Tyler Shaw (Katie) of Lexington, Jared Shaw of Lexington, Meredith Forssman (Julius) of Germany and Patrick Shaw of England; his great grandchildren, Elinor Forssman, Bennet Forssman, Harlan Hazel, Scarlett Hazel and Banks Hazel; and his brother Dick Shaw of California. He is also survived by several nieces and nephews and many other relatives and friends.
