We are sad to announce that on October 5, 2025, at the age of 26, Hazel Rakes of Campbellsville, Kentucky passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
As Hazel departs from our midst, her spirit ascends to join the cosmic dance of stars, a shimmering constellation of memories that will forever grace the night sky of the hearts of those who loved Hazel. Though Hazel may no longer walk beside us, her legacy lives on — a testament to the enduring power of love to transcend the boundaries of time and space.
She was predeceased by: her mother Jennifer Gail Rakes (Tungate). She is survived by: her father Robert Lee Rakes (Jennie Colvin) of Campbellsville; her children, Jessica Hope Smothers, Oliver Ellis Hall and Anderson Blake Hall; and her grandmothers, Linda Lou Rakes and Judy Tungate.
Visitation will be held on Thursday, October 9th 2025 from 11:00 AM to 1:00 PM at the L. R. Petty Funeral Home and Cremation Service (1765 New Columbia Rd, Campbellsville, KY 42718). A funeral service will be held on Thursday, October 9th 2025 at 1:00 PM at the same location.
