He is survived by: his son Shannon Wayne Underwood of Ashland; and his close friends, Dallas Payton of Campbellsville, Amy Watson of Campbellsville, Shawn Myers of Campbellsville and Tim Akin of Campbellsville. He is also survived by other relatives and friends.
