He is survived by: his sons, Jack Hazelwood, Jr. (Peggy) and Cary Hazelwood (Danielle); and his grandchildren, Jacob Hazelwood and Jillian Macdonald (Kyle). He is also survived by numerous nieces, nephews, grandnieces and grandnephews.
