He is survived by: his wife Jewell Frazier Crislip; his grandson Okie Frazier IV; his children, Dannie Ray Crislip (Stephanie Stephens) of Catlettsburg and Shawn Marie Blaylock of Beaumont, Texas; and his siblings, Cecil Crislip Jr. of Ashland and Catherine Daniels of Ashland, KY. He is also survived by several extended family members and friends.
