He was predeceased by: his parents, Albert Horn and Cora Gibson. He is survived by: his wife Linda Horn; his sons, James Horn (Jessica) of Bardstown and Jeff Horn (Ali) of Elizabethtown; his brother Gary Chafin of Eastview; his grandchildren, Amber, Autumn, Kali, Owen, Taylor and Brooklyn; his great grandchild Lincoln; and his daughter Melissa Peach.
