He was predeceased by: his parents, Cecil Cook and Catherine Pryor Cook; his former wife Eudena White Cook; his children, Darrell "Pete" Cook and Denise Johnson; his brothers, Denzil and Delmar Cook; and his stepchildren, David McKinney and Kerry Allen McKinney.
He is survived by: his wife Betty White Cook of Morgantown; his son Steve Cook (Melissa) of Cromwell, Kentucky; his daughter-in-law Tina Cook of Cromwell, Kentucky; his grandchildren, Amber McIntosh (Travis) of Morgantown, Luke Willis (Reva) of Livermore and Sam Cook; and his stepson Jerry Scott McKinney (Aida) of Texas. He is also survived by Horse Branch, Kentucky; seven great-grandchildren.
