He was predeceased by: his parents, Pauline Qualls Hayes and Johnny F. Hayes; his wife Ginger Shelton Hayes; his son Maurice Chan Hayes; his grandson Taroh Dominick Negyen; and his siblings, John Hayes, Jimmy Ray Hayes, Wayne Hayes, Helen Sue Myers and Evelyn Perkins.
He is survived by: his children, Thomas Trent Hayes (Kelly) of Madisonville, KY, Terry Hayes (Mary Jane) of St. Charles, KY, John Hayes of Grand Junction, CO and Chandra Beck (Allen) of Evansville, IN; his siblings, Jo Ann Ashby of Kuttawa, KY and Kenny Hayes (Ann) of Madisonville; and his grandchildren, Ramachi Hayes of Grand Junction, CO, Zach Hayes (Rachel) of Clarksville, TN, Adelene Beck of Evansville, IN, Elise Beck of Evansville, IN and Ann Marie Holmes (Tyran) of Hopkinsville, KY. He is also survived by several nieces and nephews.
In lieu of flowers, the family asks that contributions be made towards Bro. Tommy's and Ginger's monument.
