He was predeceased by: his parents, Phillip Perkins and Wanda Judd Perkins; and his grandparents, Robert Judd and Lera Katherine Hurt. He is survived by: his wife Jacqueline Perkins of Edmonton; his children, Karli Perkins, Danya Perkins (Nick Bryant), Auna Lessenberry (Richard Slater) and Khloe Westmoreland; his grandchildren, Brayden, Bryson, Remi and Koda; his siblings, Lisa Miles (James Earl), Lori Sturgeon (Robert) and Ricky Perkins; and his close friend Max. He is also survived by aunts, uncles, nieces and nephews.
