He was predeceased by: his parents, William Smith and Charlotte Smith. He is survived by: his wife Lynn Smith; his daughter April Johnson; his grandsons, Devin Justice, Joshua Justice and Jordan Johnson; and his brothers, Jeff Smith (Gail) and Steve Smith (Kerri). He is also survived by several nieces and nephews.
In lieu of flowers, memorial contributions may be made to St. Jude Children's Research Hospital, the American Cancer Society, Kenton County Animal Shelter.
