He was predeceased by: his parents, Alvin Perry and Mildred Lee Ridell Smith; his siblings, Gene Smith and Gloria Roberts; and his grandchildren, Ashley Huckaby, Kayla Taylor, Amber Hayes and Gabriel Stacey.
He is survived by: his children, Derek Smith (Jennifer) of Unionville, TN, Jeff Price (Jamie) of Independence, KY, Sonya Blackwell (David) of Murfreesboro, TN, Sharon Hileman (Eric) of Columbia, TN and Crystal Tucker of Summertown, TN; and his sister Joyce Smith of Verona, KY. He is also survived by 11 grandchildren and eight great-grandchildren.
