He was predeceased by: his mother Terri Cooper; and his grandmother Terri Nichols. He is survived by: his father Lawrence Hornsby; his wife Samantha Hornsby; his sons, Peytton Hornsby, Eric Hornsby and Steven "Monkey" Hornsby; and his siblings, Michael Hornsby (Sheila) and Jessica Fausz. He is also survived by many other nieces, nephews and friends.
