He was predeceased by: his parents, Delmer Baker and Phyllis Baker. He is survived by: his significant other Joyce Baker; his daughter Jennifer Baker Hazel (Todd); his grandchildren, Ben and Maddox; his great grandchild Eric Baker (Laura); his sisters, Pam and Kelly; Cash and Ava.
In honor of the outstanding care he received at Hospice House, donations may be made to Hospice of Southern Kentucky in his name.
