He is survived by: his wife Linda Lee Sergent Cook; his sons, Billy "Pete" Cook (Leonetta) and John Cook (Melissa); his granddaughters, Carmen Cook (Michael) and Caitlyn Sims (Stephen); his great grandchildren, Jasper and Adalynn "Ada"; his siblings, Charlene Cook Risner, Drema Cook Wilson (Ricky) and Marty Dean Cook (Elizabeth); and his brothers-in-law, Delmas Martin and Robert Bates.
Memorial Contributions can be made to the Appalachian Community Relief Fund and St. Jude in honor of William.
