He is survived by: his daughters, Anne Dvorak Howell (Christopher) of Harrodsburg, KY and Jennifer Dvorak Sparks (Edward II) of St. John's, FL; his grandchildren, Jillian Howell Berry (Joshua), Jeremy Meredith Howell (Sarah), Edward Daniel Sparks III and Matthew Thomas Sparks; and his sister Dale Dvorak Hoch of Clearwater, FL.. He is also survived by 2 great-grandchildren.
