He was predeceased by: his father Jack D. Fordham, Sr.. He is survived by: his wife Stephanie Fordham; his children, Andrew Fordham and Haley Crupper (Lucas); his grandchildren, LJ Crupper and Kimora Crupper; his mother Sally Fordham; his sister Vicki Nelson (Gary); his sibling's children, Steven Bain (Savanah), Krista Childers (Glen) and Kendra Bain; and his brother-in-law Steve Bain. He is also survived by many great nieces, and nephews.
Visitation will be held on Friday, July 25th 2025 from 4:00 PM to 7:00 PM at the Kerr Brothers Funeral Home (3421 Harrodsburg Rd, Lexington, KY 40513).
In lieu of flowers, memorial contributions in Jack's memory are suggested to the American Cancer Society.
