He was predeceased by: his stepfather Terry Hart. He is survived by: his parents, Douglas Eugene Stephens of Lancaster and Diana Carol Hart (Bottom) of Harrodsburg; his daughter Kelsey Brooke Stephens (David Mudd) of Harrodsburg; and his brothers, Shannon Stephens (Billie Jo) of Harrodsburg, Jeremy Scott Stephens of Lexington, Lee Hart (Jessica) of Harrodsburg, Adam Hart (Lori) of Harrodsburg, Collin Leber of Danville and Toby Stephens of Lancaster. He is also survived by nieces, nephews, aunts and uncles.
