She was predeceased by: her parents, Ray Sebastian and Polly Herald Sebastian; her husband Dale Campbell; her sons, Kelly McIntosh, Jr. and Johnny Clay McIntosh; and her siblings, Paul David Sebastian (Norma), Fannie Ruth Colwell of Hazard and Mary Miller of Hazard.
She is survived by: her children, Steven McIntosh of Hazard and Lisa McDonald (Duncan); and her grandchildren, Josh, Caleb, Eli, Nicole, Shelby, Kayla, Courtney, Olivia, Harmony, Brittany Paige and Colton. She is also survived by Great-Grandchildren and other relatives and friends.
