He was predeceased by: his father Otto Smith; his grandparents, Robert, Alma Smith, John and Flossie Whitaker; and his aunts and uncles, Larry Smith, Ed Whitaker, Charles Whitaker, Earl Whitaker, Bud Whitaker, Emmalene Whitaker, Freda Smith, Gertrude Brewer, Lucille Smith and Eunice Smith.
He is survived by: his mother Matilda Whitaker Smith of Somerset, KY; his daughter Johnna Rae Smith of Monroe, Michigan; his sister Amanda Kay Golden (Ben); his niece Ryleigh Sophia Golden; his nephews, Elias Otto Golden and Life Companion (Autumn Shack); and his extended family, Kelly McIntosh-Butler, Deanna McIntosh and Jimmy McIntosh. He is also survived by cousins, aunts, uncles, and other relatives & friends.
