He is survived by: his mother Irene Sebastian Campbell of Hazard; his wife Tammy McIntosh of Combs, KY; his sons, Joshua McIntosh of Lexington, KY, Caleb McIntosh of Lexington, KY and Elijah McIntosh of Lexington, KY; his grandson Easton McIntosh; his stepchildren, Courtney Vanover of Combs, KY, Jordan Vanover of Combs, KY and Dylan Vanover of Combs, KY; and his siblings, Lisa McDonald (Duncan), Steven McIntosh of Combs, KY, Michelle Wentz of Ohio, Brian Cooper of Tennessee and Joe McIntosh of Tennessee. He is also survived by nieces, nephews, and other relatives and friends.
