He was predeceased by: his parents, Kelly McIntosh and Irene Sebastian McIntosh; and his brothers, Kelly'Pokey' and Johnny McIntosh. He is survived by: his wife Chastain McIntosh of Nicholasville, KY; his children, Brittany McIntosh of Campbellsville, KY and Colten McIntosh of Bardstown; and his siblings, Lisa McDonald'Duncan' of Versailles, KY, Michelle Wentz'Jason' of Huron, OH, Joe McIntosh and Brian Cooper.
