He is survived by: his mother Melissa Maggard (Gary Neace); his children, Caeden Sebastian of Hazard, Parker Sebastian of Hazard and Autumn Sebastian of Hazard; his siblings, Timothy Shawn Sebastian (Tara), James Michael Sebastian (Savannah) and Brittnay Nashae Keene (Jon); his aunts, Trish Godsey, Bertha Sebastian and Polly Sebastian; and his uncle Michael Ray Caudill.
