He was predeceased by: his father Dwight Zane Ballard; and his stepfather Kurt Dold. He is survived by: his children, Amber Phoebe Barnes (Dylan) of Morganfield, KY and Andrew Ballard (Kendall) of Chattanooga, TN; his mother Dolly Dee Dold Ritter (Merle) of Indianapolis, IN; and his siblings, Cathy Lynn Dutt (John) of Rockwell, TX and Keith Ballard (Wanda) of Henderson, KY. He is also survived by several nieces and nephews.
