He is survived by: his children, William "Bill" Smith of Hopkinsville, KY, Michelle Waggoner (Philip) of Hopkinsville, KY, Phillip David Smith, Sr. of Sharon Grove, KY and Kelly Shelton (Eric) of Greenville, KY. He is also survived by twelve grandchildren, and several great grandchildren.
