He is survived by: his mother Flora Davis Puckett of Jackson, KY.; his sisters, Nora Herald (Stephen) of Jackson, KY and Desra Franks of Nicholasville, KY.; his nephews, Brandon Moore and Ryan Herald; and his niece Alyssa Moore. He is also survived by other relatives and friends.
