He was predeceased by : his parents, Kenneth Chism and Margaret Chism; and his stepdaughter Diana Wigginton. He is survived by : his wife Shirley Chism; his son Michael Chism (Crystal); his stepdaughters, Donna Smith and Robyn Gordon Rakestraw; his grandchildren, Roman, Max, Archer, Kristin, Lauren, Natalie, Spencer and Ashley; his sisters, Kay Human (Andy) and Patti Dew; and his friends, Linda and Tony Osting.

Memorial donations can be directed in Tom's name to Southern High School Class of 1959 Scholarship Fund in Louisville.
