He was predeceased by : his parents, Walton Fairbanks Forgy and Lurlean DeArmond Forgy; and his sisters, Mary Shackleford and Avis Jean Richmond. He is survived by : his wife Donna Forgy of Lewisburg; his daughter Janice A. Forgy of Lewisburg; his siblings, Janice D. Forgy of Adairville and Vernice Forgy of Lewisburg; and his grandchildren, Gabriel Forgy of Lewisburg and Ilana Duncan of Russellville.
