He was predeceased by: his parents, William F. Clemons (Mickey) and Besline Boyd; his sister Elaine Clemons; and his wife Shirley Clemons. He is survived by: his siblings, Bill Clemons (Phyllis) of Sharpsburg, KY, Charlotte Powell (Moses), Nannie Drakes of Dayton, OH and Brenda Turley of Lexington, KY; his children, Sherilyn Scott-Blackburn (Kirk), Katrina James (Gary), Lekeisha Ragland (Tony), Jack Clemons Jr. and Hazel Sexton; his grandchildren, Dainisha Scott, Tra'Sean James, Marquise James and Taylor Blackburn; Prince, Caspian, Maclen, Hollis and Remy. He is also survived by nieces, nephews, cousins and friends.
