He was predeceased by: his parents, Charles Cook and Sudie Cook of Elys Holler; and his daughter Malinda Lawson of Pineville, KY.. He is survived by: his wife Kathy Cook of London, KY; and his children, Joe Cook (Judie) of Middlesboro, KY, Kathy Miller (Johnny) of Pineville, KY, Van Carnes (Misty) of Cleves, OH, Laura Vandermark (Jerry) of Williamsburg, VA and Kim Lawson (Brian) of Wise, VA. He is also survived by thirteen grandchildren; ten great grandchildren.
