He is survived by: his sons, Jeremy Forman (Lynn) and Andrew Forman (Courtney); his grandchildren, Kat Kreimer, Cambria Forman, Colton Forman, Andi Grace Forman, Elijah Forman and Adaleigh Forman; his close friend Wanda Gilbert; and his siblings, Freida Bellew, Billie Jo Smallwood (Jamie) and Velman Forman.
