She was predeceased by: her parents, Ray E. Spade and Hazel Spade; her husband Lewis Thomas "L. T." Lewis; her sisters, Phyllis Hollandsworth and Ruth Myles; her brother-in-law Joe Myles; her brother Ray "Babe" Spade; her sister-in-law Norma Spade; her brother Charles Spade; her siblings-in-law, Pauline Spade, Carol Spade, Paul Lewis and Jane Sherman; and her pet Koala.
She is survived by: her daughter Karen Sue Harrod (Mike) of Bagdad; her stepgranddaughter Hannah Harrod of Louisville; her pet Empty of Bagdad; her daughter Margaret Ann Lewis of Jeffersonville; her brothers, Tommy Spade of Middlesboro and Bruce Spade (Debbie) of Wise; and her siblings-in-law, Jack Hollandsworth (Virginia) of Ewing and Bonnie Lewis of Cleveland. She is also survived by many nieces, nephews and great-nieces and great-nephews.
In lieu of flowers, the family has requested that donations be made to Samaritan's Purse, PO Box 3000, Boone, N.C., 28607-3000 or to Tates TNR and Rescue, 411 North 15th Street, Middlesboro, KY, 40965.
