He is survived by: his children, Casey Lee Lewis (Amber) of London, Kentucky and Heather Marie Lewis (Don Birkbeck) of Barboursville; his great grandchild Zaylee Jean; his close friend Cheryl Kelsey of Morehead, Kentucky.; his sisters, Judy Buelterman (Bill) of Morehead, Kentucky and Kathy Trent of Morehead, Kentucky; and the mother of his children Marlene Holbrook of Elliott County. He is also survived by several nieces and nephews; and other extended family and friends.
