He is survived by: his children, Jeffery Smith and Tammy Collinsworth; his sisters, Emmie Vance and Zeda Amburgey; his grandchildren, Rebecca Dawn Hayes (Rod), Amber Nicole Collinsworth (Jayson Hale) and Cameron Smith; his great grandchildren, Jeffrey Caleb Combs (Brandi Amburgey), Zackrey Scott King and Alyssa Kelsey Dawn Campbell; his great great grandchild Nicolas Axel Colwell; and his pet Penelope Jane.
Visitation will be held on Wednesday, October 15th 2025 from 6:00 PM to 10:00 PM and on Thursday, October 16th 2025 from 10:00 AM to 10:00 PM at the Nelson-Frazier Funeral Home - Hindman Chapel (125 Parks Rd, Hindman, KY 41822). A funeral service will be held on Thursday, October 16th 2025 at 7:00 PM and on Friday, October 17th 2025 at 1:00 PM.
