He is survived by: his wife Linda Lewis; his children, Denise Cybulski (Jason) and Jerry Lewis (April); his grandchildren, Blake Goins, Hannah Hayden (Luke), Kyndall Cybulski and Josh Lewis; and his great grandchild Hank Hayden. He is also survived by several nieces and nephews.
