He was predeceased by: his parents, J. D. McIntosh and Sue Cook McIntosh; his daughter Lori McIntosh Smith; and his brother Ronnie McIntosh. He is survived by: his wife Madeline; his sons, Jay McIntosh (Kim) of Houston and Josh McIntosh (Laura Lou) of Kevil, Kentucky; his siblings, John McIntosh (Becky) of Paducah, Kentucky and Brenda Milam (Daniel) of Paducah, Kentucky; his granddaughter Leah; and his great grandchildren, Ben, Cody, Chloe, Makenna and Sully.
Tommy Cook McIntosh, age 79, departed this life at 8:14 p.m. on Friday, November 18, 2022 following a battle with Alzheimer's Disease.
