He is survived by : his mother Vickie Banta (Gary); his sister Amanda Dees (Jeff); his nephew Aidan Dees; his niece Avery Dees; and his aunts and uncles, Pat Smith, Phillip Middleton (Lisa), Mike Middleton (Martha), Donnie Middleton (Suellen), Lori Slone (Winson) and Linda Nelson.
