He was predeceased by: his parents, John Dils Forsyth Sr. and Jean Clair Moon Forsyth; and his siblings, Augustus Preston "Buddy" Forsyth, John Dils Forsyth Jr., James Price "Jim" Forsyth, Linda Grace Forsyth, Sylvia Marie Forsyth and Rosa Lee Forsyth.
He is survived by: his wife Jeanette Karen Thomas Forsyth; and his daughters, Salena Sims (Brian) of Winchester, Tina Forsyth of Pikeville, Danette Biliter of Pikeville and Angela Gross of Pikeville, Kentucky.. He is also survived by twelve grandchildren and nineteen great-grandchildren.
