He was predeceased by: his father William Collins. He is survived by: his mother Rebecca Kendrick; his wife Monica Wallen Collins of Pikeville; his sons, Nathan Collins (Alexis Williams) of Paintsville and Braxton Collins (Sarah Hunt) of Martin; his sister Amanda Luna (Danny) of Prestonsburg; and his grandchildren, Wesley, Elizah and Beckett.
