He is survived by: his wife Judy Carolyn Glass Creekmur; his children, Molly Forsythe (Chris) of Murray and Mark Creekmur (Jenny) of Murfreesboro, Tennessee.; his grandchildren, Warren Christopher Forsythe (Sarah), Kathryn Elizabeth Forsythe, Blake Ericson Creekmur, Katie and Brett Anderson Creekmur; and his great grandchildren, Jericho Christopher Forsythe and Lily Grace Forsythe.
