He was predeceased by: his grandfather Phillip Lewis Sheets; his parents, Donald Harry and Katherine Marie Sheets; his wife Linda Kay Sheets; his sister Elissa Nichols (Jim); his son Jonathan P Sheets; and his grandchildren, Mary Elizabeth Frey and Nathan Lewis Frey.

He is survived by: his daughter Lee Ann Frey (David); his grandchildren, Matthew Frey (Ashley), Zachary Sheets (Emily) and Alison Sherman; and his daughter-in-law Trish Blair.

In lieu of flowers, contributions can be made to Hosparus of Louisville.
