She is survived by: her siblings, Paul Anthony Cook (Theresa), Danny Cox (Dawn), Carolyn Cox and Jennifer Seaton (Aaron); and her pets, Levi and Angel. She is also survived by many nieces, nephews, cousins, extended family and friends.
The family requests that contributions in Amy's memory be made to The Missions Fund at Christ Community Church.
