He was predeceased by: his grandchildren, Simmons and Martha (James Perkins). He is survived by: his parents, George Perkins and Sandy Perkins; his siblings, Brad Perkins and Melissa Williams; and his children, Austin Hoffman, Sean, Damon Smith, Tristan Porter and Summer Hodge. He is also survived by 2 grandchildren; and 5 nieces and nephews.
Visitation will be held on Friday, July 18th 2025 from 12:00 PM to 4:00 PM at the Hardy-Close Funeral Home (285 S Buckman St, Shepherdsville, KY 40165). A funeral service will be held on Friday, July 18th 2025 at 4:00 PM at the Chapel of Hardy-Close Funeral Home (285 S Buckman St, Shepherdsville, KY 40165).
The family has asked in lieu of flowers for donations to be made to Hardy-Close Funeral Home to help with the funeral expenses.
