He was predeceased by: his parents, Cecil Phelps and Emogene Smith Phelps. He is survived by: his wife Lavonna Potter Phelps; his children, Jason Phelps (Charissa) of Eubank, KY and Amy Hoard (Brian) of Versailles, KY; his sister Elizabeth Phelps Brown (Bruce) of Somerset, KY; and his grandsons, Lincoln Phelps, Christopher Hoard and Connor Hoard.
In lieu of flowers, the family requests that donations be made to the United Service Organization (U.S.O.).
