He was predeceased by: his parents, Jesse Cook and Magdaline Cook; his in-laws, Ralph and Donna Mounce Bullock; and his siblings, Van Roe Cook, Jesse "Junior" Cook, James "Shello" Cook, Arthur "Dedarde" Cook, Bobby "Bubbus" Cook, Sally "Soddie" Boyd, Gracie Jean Dobkins and Debbie Correll.
He is survived by: his wife Rita Gail Bullock; and his siblings, Butch Cook of Somerset and Ruby Goff of Somerset. He is also survived by nieces, nephews and friends.
