He is survived by: his mother Barbara Lewis of Joliet, IL; his wife Julie Mayfield; his son Kristopher Mayfield (Brenda); his grandson Joe Tucker (Jenn); his daughters, Helen Weddle (Barry) and Carrie Miner (Eddie); his siblings, Brian Mayfield (Nancy), Joe Mayfield (Jen), Christopher Mayfield, Cindy Hendricks (Tim) of Illinois and Julie Martinez (Jesse) of Illinois; and his goddaughter Hailee. He is also survived by 7 grandchildren.
