She is survived by: her children, Jeffrey Leon Forker (Sarah) of Madisonville, KY., Melissa Cowan (Ronnie) of Sebree, KY and Pam Nelson (Pastor Carl). She is also survived by 6 Grandchildren 10 Great Grandchildren 2 Great-Great Grandchildren.
