He was predeceased by: his parents, Grace Scott (Turner) and Sam Claude Scott; and his siblings, Dorothy Walker, Freddie, Charles and William Scott.

He is survived by: his wife Juanita Scott of Tompkinsville, KY; his daughters, Shauna Scott-Bellaccomo, Barbara Reddick of Edmonton, KY and Beverly Mills of Chesterfield, NJ; and his sister Mary Humes of Tompkinsville, KY.. He is also survived by five grandchildren and two great grandchildren.

In lieu of flowers the family has asked for donations to be made to the Shanti Niketan Hospice Home in Glasgow, KY in remembrance of Jack.
