He was predeceased by: his wife Donna Cook. He is survived by: his children, Denise Williams (Tom), Delene Branstutter (Frank) and Darin Cook (Karen); his grandchildren, Lauren Chen (Tommy), Lindsey Fields (Taylor), Alexis Williams, Alivia Johnson (Tommy), Aleaha Cook, Ava Cook, Jaxon Nienaber and Brooklyn Coyle; his great grandchildren, Lucas, Isaac and Sutton Fields; and his sister Katherine Ann Hein.
