He was predeceased by: his parents, Roscoe Hornback and Joan Hall Hornback; and his siblings, Ralph and Elaine. He is survived by: his wife Elaine Bengfort Hornback; his son Dr. Brett Hornback (Leslie); his grandchildren, Quinn, Spencer Hornback, Liam, McKenna Mullican, Haley, Dugan and Taylor Hauser; and his brother Phillip Andrew Hornback (Patty).
