He was predeceased by: his parents, Roy Otis and Dorris Gifford Blake. He is survived by: his wife Terri Blake; his daughter Shanna Blake Collins (Arnold) of Warfield; his grandchildren, Brady Newsome and Charlee Newsome; and his brothers, Roy Blake (Kathy) of Chesapeake, Darrell Blake of Chesapeake, Ralph Blake of Chesapeake and David Blake (Sherry) of Papinaw. He is also survived by sisters-in-law, brothers-in-law, nieces and nephews.
Visitation will be held on Monday, December 15th 2025 from 6:00 PM to 9:00 PM at the Mullins Family Funeral Home (106 Riverfront Road, Warfield, KY 41267). A funeral service will be held on Tuesday, December 16th 2025 at 12:00 PM at the same location.
